APPLICATION FOR WATER OR USE OF SANITARY SEWERS

I hereby make application for use of the water system and/or sanitary sewer and agree to be governed by the Rates, Rules, and Regulations as adopted by the WTWSD for water and sanitary sewer, which are made part of the application with the same effect as if physically attached to it, and further agree that I shall not permit a sump pump or roof drainage to be connected to or enter the sanitary sewer system from the described premises.

I agree to notify the WTWSD inspector at 215-343-1800 when the building sewer (from the building to the service line connection) is ready for inspection and connection to the public sewer, but before any portion of the work is covered.
